Plan Ahead for Your Healthcare Wishes

April 16th is National Healthcare Decisions Day, and we at Capital Health Plan (CHP) want to encourage everyone to think about their healthcare choices. It’s a great time to talk about what you want for your health in the future.

Advanced care planning isn’t about old age or illness. It’s about planning for the future and making decisions about preferences. The Five Wishes document is simple to complete, can be changed as often as desired, and only goes into effect if you are incapacitated and unable to make decisions for yourself.
