Service Area
Capital Health Plan provides comprehensive and affordable health care to our members in the Tallahassee area. We proudly serve Calhoun, Franklin, Gadsden, Jefferson, Leon, Liberty, Madison, Taylor, and Wakulla counties in Florida.
Eligibility
Capital Health Plan offers Employer Sponsored benefit plans to large and small employer groups located in the CHP service area. In addition to those who are Medicare eligible (Medicare products are not currently available in Madison and Taylor counties), additionally we offer Individual and Family plans through the Federally Facilitated Healthcare Exchange.
