Request a Proposal

Use the following forms to request a proposal. Make sure to use the correct forms for your client’s group size, as the information required depends on the number of people in the group.

Small Employer Group: 1-50 Employees (total average number including full-time, part-time, and seasonal employees):

Large Employer Group: 51 or More Employees (total average number including full-time, part-time, and seasonal employees):

New and Renewal Business

New Business

Capital Health Plan offers employer-sponsored benefit plans to both large and small employer groups located in the CHP service area. Available benefit plans vary depending on the group size. Employer groups with 50 employees or less are considered to be a Small Employer Group, whereas those with 51 or more employees are considerd to be a Large Employer Group.

CHP will determine which category the employer group is in based on the total average number of employees a group had during the preceding calendar year. This includes full-time, part-time, and seasonal employees.

Renewing Business

Capital Health Plan will mail the group a renewal packet at least 45 days before the Employer Group's renewal date. This packet will include information regarding rates and benefit plans, along with instructions for completing the renewal. The agent copy will be placed on your account via CHPConnect.
