New Member Information

We understand that getting started with your health plan can be a little confusing. Below we’ve put together a few resources to help you get started as a CHP Member:

Sign up for CHPConnect

As a new CHP member, you will need to create an account for CHPConnect, our convenient online member portal where you can manage many aspects of your health care. Members have the ability to:

  • Review/download benefit documents
  • Choose a Primary Care Physician (PCP)
  • Review lab results
  • Request ID cards
  • Review referrals and authorizations

Select a Primary Care Physician

Our new member checklist guides you through the process of how to choose your CHP primary care physician (PCP), as one of your first steps as a CHP member is selecting a PCP. Thanks to our large doctor network, you may even discover that the doctor you know and trust happens to be one of our in-network providers. Your PCP is your healthcare partner and central coordinator for everything pertaining to your good health. Primary care physicians specialize as family medicine doctors, internal medicine physicians, or pediatricians.

Discover Our State-of-The-Art Health Centers

In addition to providing coverage for healthcare within our service area of Tallahassee and the surrounding regions, CHP operates three member-exclusive CHP Health Centers. Members can find a PCP at any of our three health center locations. CHP’s Health Centers offer routine care services such as X-rays, eye exams, screening tests, and much more.

Access Your Benefit Documents

To access your CHP benefit documents 24/7, simply scan the QR code found on the back of your CHP member ID card or visit CHPConnect.

For additional assistance, you can reach out to your benefit administrator or call CHP Member Services to have your documents sent to you by mail.

Learn How to Submit Claims

Your doctor or provider will submit claims for covered services on your behalf, but you must present your CHP member ID card. You can view your claim information via CHPConnect for 24/7 access or you may call CHP Member Services.

Utilization Management/Evaluation of New Technology

CHP conducts regular evaluations of emerging medical technologies, including drugs, procedures, and devices, to determine coverage eligibility. Frequently Asked Questions By New Members

I am a new member. When will I receive my ID Card?

You should receive your Member ID card within 10 business days of CHP’s receipt of a complete and accurate enrollment application.

I am a new member. Can I receive care before I get my ID card?

Yes. You may print a temporary Member ID card on the CHPConnect portal or contact CHP Member Services to request your Member ID number. Additionally, you can provide your name and date of birth to your Provider and they can contact CHP to verify eligibility and copayment information on your behalf.

What is a primary care physician (PCP)?

A PCP is a physician you select to serve as your primary physician to maintain your needs. They handle a variety of health-related problems, keep a medical history and medical records, and can help coordinate care with a specialist or other health care professional.

What services are offered at the Physician Group of CHP?

The Physician Group of Capital Health Plan provides comprehensive health care exclusively for Capital Health Plan members. Their goal is to promote disease prevention and healthy living, so in addition to diagnosing and treating illnesses, they also provide preventive care, health risk assessments, immunizations, screening tests, comprehensive management of chronic conditions, and education on maintaining a healthy lifestyle.
