Eligibility
Capital Health Plan offers Employer Sponsored benefit plans to large and small employer groups located in the CHP service area. In addition, CHP has Medicare Advantage plans available to those who are Medicare eligible which are individuals nearing 65, already 65, or over 65. CHP also offers Individual and Family health plans through the Federally Facilitated Healthcare Exchange.
