According to the U.S. Census Bureau, within the next ten years, one in five residents will be of retirement age. This population has greater medical complexity – about 75% have at least two chronic conditions and over 40% are taking five or more medications. Recognizing the unique needs of our aging population, Capital Health Plan proactively established the Nancy Van Vessem, M.D., Center for Healthy Aging at the CHP Metropolitan Health Center.

Our physicians are Internal Medicine or Geriatric physicians dedicated to improving health through evidence-based, Age-Friendly health care for older adult members.

Leading the Way in Age-Friendly Care

In collaboration with the John A. Hartford Foundation and the Institute for Healthcare Improvement, CHP has been recognized as an Age-Friendly Health System – Committed to Excellence. CHP is an early adopter of the philosophies of this movement which provides care for older adults based on an essential set of evidence-based practices in a framework known as the 4M’s.

  • What Matters: Understanding the personal goals and priorities of older adults to tailor care.
  • Medications: Reducing or eliminating medications that may be harmful or no longer needed.
  • Mentation: Focusing on cognitive health to prevent, identify, and treat dementia and depression.
  • Mobility: Ensuring older adults can move safely every day, reducing falls and promoting independence.

This framework has proven to have better outcomes for patients, reducing the cost of healthcare for patients, and lowering the burden of healthcare.

As a result of our focus on long-term health outcomes with specialized support for older adults, the Nancy Van Vessem, M.D., Center for Healthy Aging was recognized as a Million Hearts® Hypertension Control Champion. These Champions demonstrated exemplary rates of hypertension control. They achieved improved performance by attending to elevated blood pressures at every visit and persisting to work with patients until blood pressure control is achieved.

Center for Healthy Aging Health Services

The Center for Healthy Aging offers the following services to meet the specific needs of older adults and adults managing complex medical conditions:

  • Primary Care services utilizing Internal Medicine or Geriatric trained physicians whose practice will be predominately-older adults, providing a comprehensive and respectful approach to medical care. Members are supported by addressing the physical, social, and emotional aspects of their medical care.
  • The Fall Prevention Clinic will assess and stratify the fall risk of older adults with a one-on-one evaluation with a trained professional
  • Cognitive Screening Clinic will provide cognitive testing using clinically validated tools by certified testers, upon referral from primary care physician.
  • Physical Therapy services utilize a specialty care provider who helps older adults stay mobile through exercise and improve overall movement.
  • Radiology Imaging Center offers digital X-ray and ultrasound services.
  • On-site Lab Services include rapid strep screen, glucose finger stick, urinalysis and more.
  • Routine Health Check-Ups: Regular health assessments to keep you on track.
  • Chronic Disease Management: Personalized care plans to manage ongoing health conditions.
  • Preventative Health Screenings: Early detection screenings to promote long-term health and preventive care to manage chronic conditions.
  • Educational Workshops and Community Health Programs: Information sessions on health, nutrition, and relevant aging topics as well as community initiatives that promotae healthy living and community engagement (Walk with a Doc).
  • Holistic Approach: Our multidisciplinary team offers comprehensive support, addressing both physical and mental well-being for a fuller, healthier life.

Dr. Nancy Van Vessem's Story

Dr. Nancy Van Vessem was a dedicated and compassionate physician who made a significant impact on the healthcare community in Tallahassee. As the Chief Medical Officer of Capital Health Plan, she was known for her unwavering commitment to improving healthcare quality and accessibility for all. Tragically, Dr. Van Vessem lost her life in a yoga studio shooting in Tallahassee in 2018. Despite this heartbreaking event, her legacy continues to inspire and guide us. Dr. Van Vessem’s vision and leadership were instrumental in shaping the Nancy Van Vessem, M.D., Center for Healthy Aging, which continues to carry forward her legacy of excellence in care and innovation in health services. Her work has left an enduring mark on our community, and we are proud to honor her memory through the center that bears her name.

Disaster Recovery

The safety and well-being of our patients is a top priority, along with environmental sustainability. The CHP Metropolitan Health Center in Tallahassee is equipped with an energy-efficient cooling system and utilizes the community grid power during off-peak periods. The entire building is powered by a generator, so it never loses power during an outage – keeping the safety and well-being of our members as a top priority while remaining environmentally sustainable.
