The Medication Therapy Management Program (MTMP) works to identify members at risk for potential drug-related problems and helps these members better understand the drugs they take. Some members have several complex medical conditions or they need to take many drugs at the same time, or they may have very high drug costs. This program helps members manage their drugs and helps reduce potential problems. The Medication Therapy Management Program can help you and your doctors get the most benefit from your drugs.
Who qualifies for MTMP?
If you qualify, this program is offered at no cost to you.
You will be automatically enrolled in this program if you:
- Take eight or more Medicare Part D-covered maintenance drugs, and
- Have three or more long-term health conditions, such as diabetes, high blood pressure, chronic heart failure, or high blood cholesterol levels.
- Spend more than $5,330 a year on your drugs
How will I know if I qualify?
It's easy to get started. If you meet the criteria above, you are automatically enrolled and will receive a Participation Form in the mail. To receive interactive services from the MTMP, complete and return the Participation Form. Or, you may call the phone number on the offer letter and ask to speak to someone about the MTMP. We hope you will participate.
There is no added cost for this program and you may leave the MTMP at any time. Your choice whether to take part — or not — has no effect on your insurance benefit.
How does it work?
The Medications Therapy Management Program, offered by your Prescription Drug Plan, allows you talk to a pharmacist or trained MTM nurse about your medicines. The MTMP can help you get the most from your medicines.
MTMP pharmacists and trained MTM nurses can answer your questions about prescription medicines and over-the-counter products. MTMP services include:
- Review of your medicines
- Personalized Medicine List
- Help in understanding how your medicines work and their side effects
- Educational materials
