How to get the most out of our benefits

Pick a network PCP

A PCP is your primary care doctor or provider, sometimes called a primary care physician or doctor. It can be a family practitioner, internist, pediatrician, or general medicine physician. Your PCP generally:

  • Knows your history
  • Builds an in-depth knowledge of your health over time
  • Helps guide you on the best path of care
  • Can advise you when to see a specialist and provide prior authorizations or referrals, if necessary

Keep up on preventive care

CHP covers preventive care – such as routine wellness exams, certain recommended screenings, and immunizations – at no additional cost to you when you see network doctor or providers.

How to fill your prescriptions

Get home delivery

Order up to a three-month supply of medications you take regularly. View our mail order service for more details.

Pick up at the pharmacy

Show your CHP ID card at any of our network pharmacies – which can be found on the Pharmacy Directory.

CHP has partnered with the Tallahassee Senior Center to offer FREE on-demand virtual Mindful Movement classes on the Senior Center’s Facebook page. In addition, classes will also air on Channel 13 (WCOT) at various times and days. Visit the Tallahassee Senior Center for more information.

Care for Specific Conditions

Get emotional support

Your behavioral health benefit provides access to a network of nearby doctors and providers with options for either in-person care or a virtual visit. Get started by viewing in-network mental health professionals.

Treat chronic conditions

Capital Health Plan’s Center for Complex Care provides a comprehensive approach to the medical care of our members with chronic conditions – such as diabetes, heart failure, or coronary artery disease. Our internal medicine physicians support you with the physical, social, and emotional aspects of chronic illness to help you achieve better outcomes.

Receive free case management

CHP’s Case Management Program is a service offered to all members. Our case managers can offer assistance to members who have experienced a critical incident or have multiple complex conditions and need help navigating their care. To enroll in this free service, please call Member Services.
