What is CHP’s Fitness Reimbursement Program?

Capital Health Plan members can receive up to $150 per calendar year for membership at a qualified health and fitness centers during that year.

What is CHP’s eyeglass reimbursement program for Medicare members?

CHP’s Medicare Advantage members can be reimbursed up to $150 for eyeglasses (lenses and frames) or contact lenses every two years. Consult your plan's Evidence of Coverage for more information:

I am a Medicare Advantage member and want to take advantage of my eyeglass reimbursement. Where can I find the reimbursement form?

Medicare Advantage members may use the General Reimbursement Request Form when requesting reimbursement for eyeglasses. 

How do I get reimbursed for prescription drugs?

Non-Medicare Members: The Non-Medicare Prescription Drug Reimbursement Form should be used when requesting reimbursement for prescription drugs. Reimbursement requests can take up to 30 days for processing and may take longer if additional information is needed.

Medicare Advantage Members: The Medicare Prescription Drug Reimbursement Form should be used when requesting reimbursement for prescription drugs that our Medicare Advantage (HMO) member feels should be reimbursed. Reimbursement requests can take up to 30 days for processing and may take longer if additional information is needed. 

How is the prescription drug reimbursement processed? How much will I be reimbursed?

You must submit an itemized paid receipt and a completed Non-Medicare Prescription Drug Reimbursement Form or a Medicare Prescription Drug Reimbursement Form within 90 days of purchase. Please note that if your reimbursement is approved, the reimbursement will be made minus any applicable copayments for only the prescription drug(s) listed on the Formulary (list of covered prescription drugs). You may also contact CHP Member Services for more information.

How do I submit a request for reimbursement for medical care I paid for while out of the service area?

To submit a request for reimbursement for Covered Services, you must submit proof of payment and a completed General Reimbursement Form. Please note that if your reimbursement is approved, the reimbursement will be made minus any applicable copayments.
