I have not received my ID card. When will I get that?

Your Capital Health plan ID card has been mailed. Please allow 7-10 days to receive it. Or, you may create an account for CHPConnect, our online member portal. Once you receive your access code, you may download an ID card.

How can I get a copy of my benefit information?

If you have your ID card, turn it over to the back and scan the QR code with your phone camera; this will load your plan benefits. You can also log into your CHPConnect account and view your plan benefits. To do so click My Health Plan, then Benefits & Eligibility after you’ve logged in.

How can I select/change my Primary Care Physician?

Create an account for CHPConnect, our online member portal. Once you receive your access code, you will be able to click the appropriate link to select or change your Primary Care Physician.

I received a bill from a provider. Why am I getting this bill?

Please contact the provider office directly and make sure they have your CHP insurance information, and confirm why you have received this bill.

How can I find out if my medications are covered by Capital Health Plan?
