About This Location
At the CHP Governor’s Square Health Center, you can find physicians’ offices, X-ray and laboratory services, mammography, ultrasound, DEXA scanning, and the CHP Eye Care center. All physicians on-site and these services are available exclusively to Capital Health Plan members.
Our patients may now schedule real-time medical appointments online with their primary care doctor/provider at the Governor’s Square Health Center (Physician Group of Capital Health Plan), or with an eye care doctor/provider.

Physician Group of Capital Health Plan
At the Governor’s Square Health Center, CHP members are cared for by CHP-only physicians through the Physician Group of Capital Health Plan. Every physician is dedicated to improving your health through evidence-based, comprehensive health care for patients of all ages, genders, and conditions.
Patients seen by CHP Physicians can maintain an ongoing relationship with their doctor and practice staff through the NextGen Patient Portal. This portal is exclusive to patients of the Physician Group of Capital Health Plan and also provides access to health plan details, test results, health forms, and more.
