Physician Group of Capital Health Plan
At the Centerville Place Health Center, CHP members are cared for by CHP-only physicians through the Physician Group of Capital Health Plan. Every physician is dedicated to improving your health through evidence-based, comprehensive health care for patients of all ages, genders, and conditions.
Patients seen by CHP Physicians can maintain an ongoing relationship with their doctor and practice staff through the NextGen Patient Portal. This portal is exclusive to patients of the Physician Group of Capital Health Plan and also provides access to health plan details, test results, health forms, and more.
