What happens to my coverage if I retire?

Contact your benefits administrator. Depending on your age and employer, you may be eligible for a retiree plan or a Medicare Advantage plan. You may also call CHP Medicare Services for more information.

What happens to my coverage if I leave my job?

Contact your Employer Group Health Benefits administrator, as you may be able to continue your coverage under COBRA.

What happens to the coverage of my spouse if I divorce?

How do I get reimbursed for prescription drugs?

Non-Medicare Members: The Non-Medicare Prescription Drug Reimbursement Form should be used when requesting reimbursement for prescription drugs. Reimbursement requests can take up to 30 days for processing and may take longer if additional information is needed. You may also contact CHP Member Services for more information.

Medicare Members: The Medicare Prescription Drug Reimbursement Form should be used when requesting reimbursement for prescription drugs that our Medicare Advantage (HMO) member feels should be reimbursed. Reimbursement requests can take up to 30 days for processing and may take longer if additional information is needed.

Your benefit documents are available on the CHPConnect portal. You can request your Summary of Benefits & Coverage information from your benefits administrator, or access your Summary of Benefits & Coverage Form online for Large and Small Groups, or on CHPConnect.
