The Diabetes Disease Management Program is designed to provide resources to help members make healthy lifestyle choices while supporting the individual treatment plan. Prevention of major diabetes complications, such as stroke, heart attack, amputation, kidney failure, and blindness, is the main goal.

The Metabolic Health Center is staffed with nurses and dietitians who are certified diabetes educators. CHP members must first contact their primary care office to obtain a referral. TMH will schedule the appointment. If you have been seen at the center in the past and would like to attend again, call the number listed on the TMH website to schedule an appointment.
