Capital Health Plan Medicare Advantage (HMO) plans provide all your Medicare benefits and services, including prescription drug coverage (Medicare Parts A, B, and D). It's easy to find the plan that is right for you.
To enroll in a Capital Health Plan Medicare Advantage plan, you must be entitled to Medicare Part A and enrolled in Medicare Part B. You must be a permanent resident of our service area, which includes the Florida counties of Calhoun, Franklin, Gadsden, Jefferson, Leon, Liberty, and Wakulla. You may not be able to enroll if you have end-stage renal disease (ESRD) unless you are a Capital Health Plan member and have been since dialysis began.
You are now able to submit feedback about your Medicare health plan or other non-urgent issues directly to Medicare using the Medicare Complaint Form. Additionally, the Office of the Medicare Ombudsman (OMO) helps you with complaints, grievances, and information requests about Medicare.
