Capital Health Plan Silver Advantage (HMO) is a Medicare Advantage plan that is available to individuals who are entitled to Medicare Part A, enrolled inMedicare Part B, and live in the Capital Health Plan service area.
Capital Health Plan Advantage Plus (HMO) is a Medicare Advantage plan that is available to individuals who are entitled to Medicare Part A, enrolled in Medicare Part B, live in the Capital Health Plan service area, and do not have End Stage Renal Disease.
Capital Health Plan Preferred Advantage (HMO) is a Medicare Advantage plan that is available to individuals who are entitled to Medicare Part A, enrolled in Medicare Part B, live in the Capital Health Plan service area, and do not have End Stage Renal Disease.
Capital Health Plan Retiree Advantage (HMO) is offered by some employers to their Medicare-eligible retirees and Medicare-eligible dependents of retirees allowing members to continue to participate in their employer-sponsored health plan after retirement and becoming Medicare eligible.
Capital Health Plan Retiree Advantage (HMO) is offered by the State of Florida to their Medicare-eligible retirees and Medicare-eligible dependents of retirees. This provides our members the opportunity to participate in their State of Florida-sponsored health plan after retirement and becoming Medicare eligible.
