Capital Health Plan (CHP) is a nonprofit health maintenance organization (HMO) that has been making health care easy and affordable for people in our community for more than 27 years. That's our purpose and our passion--and these will never change.

Capital Health Plan Medicare Advantage (HMO) plans offer more than just health coverage. Being a member means that you have a committed, experienced partner to help you improve your health and quality of life.
Membership in Capital Health Plan Medicare Advantage (HMO) plans is exclusively for people entitled to Medicare Part A and enrolled in Medicare Part B who live in the Florida counties of Leon, Wakulla, Gadsden, or Jefferson. All of our plans offer more benefits than Original Medicare and cost less than many Medicare supplemental plans.
Capital Health Plan Medicare plans are designed with you in mind. Capital Health Plan is proud to provide a Medicare plan to fit your individual needs. We offer two types of plans:

Capital Health Plan Direct Pay Plans

A direct pay plan is a plan that allows the member to contract directly with Capital Health Plan to receive Medicare benefits. We offer Capital Health Plan Advantage Plus (HMO) and Capital Health Plan Preferred Advantage (HMO). Both include Medicare Part D Prescription Drug coverage.

Capital Health Plan Retiree Plans

The second type of plan is the employer-sponsored retiree coverage called Capital Health Plan Retiree Advantage (HMO). Designed and offered exclusively to Medicare retirees by some employers, Capital Health Plan Retiree Advantage allows members to participate after retirement in their employer group health plan.
