Capital Health Plan (CHP) is a nonprofit health maintenance organization (HMO) that has been making health care easy and affordable for people in our community for more than 27 years. That's our purpose and our passion--and these will never change.

CHP Medicare Advantage (HMO) plans offer more than just health coverage. Being a member means that you have a committed, experienced partner to help you improve your health and quality of life.
Membership in Capital Health Plan Medicare Advantage (HMO) plans is exclusively for people entitled to Medicare Part A and enrolled in Medicare Part B who live in the Florida counties of Leon, Wakulla, Gadsden, or Jefferson. All of our plans offer more benefits than Original Medicare and cost less than many Medicare supplemental plans.
Our Medicare plans are designed with you in mind. Capital Health Plan is proud to provide a variety of Medicare plans to fit your individual needs. We offer two types of plans:

Our Direct Pay Plans

First, we offer direct pay plans. A direct pay plan is a plan that allows the beneficiary to contract directly with CHP to receive Medicare benefits. We offer CHP Advantage Plus (HMO) and CHP Preferred Advantage (HMO). Learn more about our Medicare-approved prescription drug coverage for 2010.

Our Retiree Plans

The second type of plan is the employer-sponsored retiree coverage called Capital Health Plan Retiree Advantage (HMO). Designed and offered exclusively to Medicare retirees, CHP Retiree Advantage allows members to participate after retirement in their employer group health plan.
