Services described will be provided at one of CHP's primary care physician locations or in other designated network provider offices or in facilities authorized by a CHP physician.
Capital Health Plan provides maternity coverage without a qualifying waiting period (except to members of certain small groups). However, any costs incurred before your membership became effective are not covered by CHP.
