Capital Health Plan Is...
A federally qualified, non-profit health maintenance organization (HMO) created in 1982 to provide comprehensive and coordinated medical care for a fixed, prepaid fee with minimal (or no) copayments and no deductibles. This prepaid fee covers members' health care needs from routine physicals to hospitalization.
History
Capital Health Plan was created in 1982 and is the area's first HMO. CHP was started by local citizens concerned with the rising cost of health care and the need for an alternative system of health care delivery. Capital Health Plan is a licensed affiliate of Blue Cross Blue Shield of Florida

Plan Ratings
