CHP Health Coaching

In 2007, Capital Health Plan began offering our members an exciting new benefit -- CHP Health Coaching -- in celebration of our 25th anniversary. CHP Health Coaching is a 24-hour health information and decision support line staffed by Health Coaches who are specially trained health professionals (such as nurses, dieticians, and respiratory therapists).

Privacy Practices

All CHP member health information is confidential. Disclosures (such as employees) for purposes other than treatment, payment, or health care operations are made only when a member submits an Authorization for Release of Health Information form to CHP.

Medication Center

Capital Health Plan utilizes a Pharmacy Benefit company called Caremark to administer our pharmacy benefits. Members can register at the Caremark web-site to find out more information about their specific medications (including refill history and cost), general information on medications (side effects, storage concerns, and drug interactions), and the benefit of using generics over brand name medications.

Capital Health Plan offers our members the opportunity to access their CHP medical history, change their primary care physicians, and much more through our CHPConnect.

Member Rights & Responsibilities

Capital Health Plan (CHP) is committed to provide and/or arrange for the provision of quality health care in a cost-effective manner. Consistent with our commitment, CHP has adopted a statement of Member's Rights and Responsibilities.

Advance Directives & Living Wills

End of life decisions about medical care can be much easier when advance directives are used. An advanced directive simply means a statement, made while you are competent, about the medical treatment you want when you consciously cannot make decisions. Decisions made early and communicated plainly may have tremendous value for the person and the family.

Utilization Management

CHP makes decisions about coverage for health care services with two main goals as its focus: to cover quality treatment with the efficient use of resources. CHP's participating physicians coordinate preventive care and specialty services for their patients. CHP evaluates new technologies and develops clinical treatment tools to guide the care of members. This is called "utilization management."
